Birth to 3 Connections provides early intervention and family-centered services for young children under the age of 3 who have developmental disabilities or delays.
Services are provided in the child's and family's natural environment which may include their home and other community settings.
This program supports all Dane County communities.
Strong candidates will have direct working experience with Birth to 3, case management experience, demonstrated problem-solving skills, attention to detail, strong computer skills, and the ability to remain flexible and able to adjust to ever changing program needs and special projects.
Key responsibilities include:
Intake of newly referred families Service Coordination Requirements include:
Bachelor's degree in related field, such as social work A valid driver's license, acceptable driving record, liability coverage and a personal vehicle are required for this position.
