Social Worker - Medical Oncology

Overview Oncology department Social Worker PRN/Pool - works on an as needed basis Monday - Friday / No weekends or holidays The Social Worker BSW coordinates and facilitates patient care activities to improve continuity of care across the health care continuum, to promote appropriate utilization of resources, and to contribute to improved patient satisfaction and outcomes.
The BSW provides services and support to patients and families who are referred to Social Services or by case finding.
The BS provides discharge planning as needed.
The BSW is a strong patient advocate.
Working in collaboration with the Patient Navigator and the Multidisciplinary team, the BSW assesses and intervenes with high risk patients that require a high level of continuing care services while ensuring compliance with regulatory standards of care at the organizational, state and federal level.
As needed, the Social Worker conducts clinical review for utilization management; corresponding with payers and utilizing established criteria sets.
Responsibilities Performs home visits to patientEURs dwelling.
Investigates case situations and presents information to members of the health team on patient's family and social background pertinent to diagnosis and treatment.
Assess patients for continuing care needs, creates plan for providing continuing care, and confers with physician and team members as needed.
Refers patient or patient's family to other community resources as is indicated; assists patients and families to complete advance directives.
Educates patients about resources, options and documents all activities in a timely manner.
The BSW provides emotional support, helps patients and families understand and accept their condition as well as accepting medical recommendations; utilizes community resources to assist patients to live in the community; responsible for travel arrangements for patients; responsible for arranging changes in levels of care and settings; The hospice BSW participates in IDT meetings and is considered a core member of the Hospice team per regulations.
The home health BSW participates in team meetings regularly to provide input during patient case conferences and to educate team members regarding social work interventions that can be provided.
May be required to work flexible hours.
Education and ExperienceBachelors of Social Work (BSW) from an approved graduate school of social workHome health or hospice experience desiredOne year social work experience in a healthcare settingExperience and ability to facilitate groupsBSN or upon hire BSN completion agreement for enrollment in an accredited BSN programCertification/LicensureCurrently Licensed (LCSW/LSW) in the State of Illinois and/or WisconsinLicensed to practice in the State of Wisconsin and/or Illinois or registry eligibleApproved CPR or BLS certification required within 90 daysMaintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.
