Summary of Position We are seeking a friendly, full-time Day Services Aide to provide support and supervision to individuals in a community setting.
This position will be responsible for providing direct observation to persons served and other workers to ensure quality outcomes related to program goals and objectives.
Engage in activities that promote growth and development with the senior population with dementia and/or disabilities.
RESPONSIBILITIES Monitor Individual Service Plans and Outcome Measurement Reports to ensure the ongoing provision of quality services.
Work with seniors with dementia and/or disabilities.
Assist with planning, preparation, and maintenance of effective programming plans to include activity, exercise, and rest relevant to individual client needs and preferences.
Maintain contact with families and service providers related to the client Individual Service Plan.
Assist and supervise activities of daily living, i.
e.
, walking, grooming, toileting, eating, etc.
as needed.
Record food served at meals.
Records are to be kept on file for up to six months.
Provide snacks for those who need them.
Monitor for appropriate intake, choking concerns and other issues related to mealtime.
Provide basic first aid treatment for client illnesses and injuries that occur during the workday.
Communicate concerns to program manager and/or nurse consultant.
Ensure client illnesses and injuries that occur during the day are reported and documented.
Provide follow-up on all client illnesses and injuries that occur during the day to ensure that medical recommendations have been fulfilled, as needed Dispense medications and provide specified medical treatments in accordance with physician's orders.
Ensure written information describing side effects and adverse reactions of each prescribed medication is kept in the client's file.
Document the administration of medications.
To be kept in the client's permanent records.
Compile and maintain active and inactive case records in a confidential fashion as prescribed.
Perform crisis management and other related counseling activities.
Maintain a functional knowledge of individual client Individual Service Plans.
Assess client needs and take action to provide modifications to related objectives.
Provide supervision in activities which encourage creativity, social interaction, and physical exercise, providing opportunities for indoor and outdoor activities.
Advocate for the rights and responsibilities of those persons on assigned caseload.
Complete record keeping and reporting responsibilities in an accurate and timely manner.
Lead, facilitate and/or participate in team meetings.
Maintain effective communications with all internal and external customers.

QUALIFICATIONS Certified Nursing Assistant license from an accredited program OR applicable Caregiver experience required.
Computer Skills:
Microsoft Office and basic computer skills.
Excellent communication, both verbal and written Ability to perform basic computer skills.
This position reports to the Program Director Job Type:
Full-time Dayshift Monday thru Friday, no weekends, no evenings, no holidays.
